GLP-1 Medication Coverage Disclaimer

However, insurance coverage for GLP-1 medications is often limited, particularly when prescribed for weight management. Many insurance plans do not cover these medications, or require strict prior authorization criteria that may not be met.

For patients without insurance coverage, GLP-1 therapy may still be available through a compounded formulation. Compounded versions are typically offered at approximately one-quarter of the cost of brand-name products. These options may be purchased as individual weekly injections administered in-office or provided in vial form for self-administration, as directed by your healthcare provider.

Compounded medications are prepared by licensed compounding pharmacies and are not FDA-approved in the same manner as commercially manufactured products. Patients should discuss the risks, benefits, and suitability of compounded GLP-1 therapy with a qualified medical professional before initiating treatment.

Weight Loss Medication Program Disclaimer

For patient safety and appropriate medical management, all patients starting weight loss medications are required to have laboratory testing completed within the past three (3) months prior to initiating treatment.

If recent laboratory results are not available at the time of your appointment, laboratory services will be performed in-office before medication can be prescribed. These services are covered by most insurance plans; however, patients are responsible for verifying individual coverage and any applicable copays or deductibles.

The initial appointment will include a comprehensive assessment and evaluation with the provider to determine medical eligibility and establish a treatment plan.

All follow-up visits up to the three-month mark, as well as routine medication injections, will be scheduled as nurse-only visits unless otherwise requested by the patient or deemed medically necessary by the provider.

If a patient requests to see the provider during a scheduled nurse visit, the appointment will be billed as a standard provider visit in addition to the cost of the medication administered.

Participation in the weight loss program indicates acknowledgment and agreement with these policies.
